REGISTERED PRACTICAL NURSE

Bargaining Unit: UNIFOR-HOSPITAL
Status: Part Time (0.6 FTE)
Department: Seniors Outpatient Services
Site: St. Joseph's Hospital
Shifts: Days
Rate of Pay: $29.89 - $30.27 per hour

As part of Seniors Outpatient Services, the Registered Practical Nurse provides direct and indirect comprehensive service and support to geriatric clients, care partners and the community as a member of the inter-professional team. The RPN provides ongoing client health monitoring, therapeutic interventions, and coaching, mentoring and support to care partners in employing new strategies when working with clients that may be displaying responsive behaviours.

Qualifications:
- Current Registration and in good standing with the College of Nurses of Ontario. Valid CPR certificate
- Minimum of three years nursing experience working with older adults with Dementia, Mental Health, and other Neurological Conditions, preferably in a community setting
- Demonstrates the knowledge of dementia, delirium, mental health issues in the delivery of care and its effect on the person and family members or care partners
- Ability to assume responsibility for an assigned client caseload
- Experience in health and behavioural assessment, symptom management, medication assessment and monitoring, health teaching, and supportive counseling
- Knowledge and experience with various legislation including the Mental Health Act, Health Care Consent Act, and other relevant legislation
- Demonstrated ability to problem solve using sound judgment in applying critical thinking skills within safe limits of client care
- Ability to work independently and collaboratively within an inter-professional team
- Ability to communicate clearly and concisely, both verbally and in writing
- Extensive knowledge of local and regional community resources
- Computer literacy in accessing electronic medical records, electronic documentation, and e-mail
- Serve as a resource for clients and professional colleagues
- Demonstrated regular attendance at work
